KOOTASCA Community Action - Community Needs Assessment Household Survey 2025-2026

KOOTASCA Community Action is conducting its 2026 Community Needs Assessment to better understand the challenges faced by low-income households in Itasca and Koochiching Counties. If your household sometimes struggles to make ends meet—whether with housing, childcare, transportation, health, or other basic needs—we invite you to share your experiences.Your input will help shape KOOTASCA’s programs and ensure services reflect the real needs of families in our community. This survey is intended for low-income households. Non-low-income community leaders and partners will have a separate opportunity to provide input later in the process. The survey takes about 10–15 minutes to complete, and all responses are confidential. As a thank-you, you may enter a drawing for one $250 grand prize and ten $100 prizes after completing the survey via a short, separate prize-entry form. One entry is allowed per household. Winners will be drawn when the survey period closes in April 2026.
